    Add player.txt to installer manifest and README

    player.txt (Player Reference Manual) was added in 1df3310153 but not
    included in install.ini or the README doc table. Add explicit entries
    in both :install.files and :upgrade.files sections of install.ini, and
    add the Player row to the documentation table in README.md.
